How to Get a HCP Physiotherapist
Check your Eligibility
The Aged Care Assessment Team (ACAT) will assess your eligibility. Assessment is based on Care needs and supports required to maintain home independance.
HCP Approval
When your HCP package level is approved, a HCP provider will assist you in managing the care you need.
Home Care Provider Assessment
Your Home Care provider will then be in contact with us and determine the support required based on your care needs
Our team will come to you!
On Approval from your Home Care Provider, our Physiotherapy team will come to your doorstep, and provide the support that you need, based on your goals!
